Does a doctor monitor the Symptom Tracker?

No, a doctor does not monitor this application.

This application does not provide, and is not a substitute for, medical advice, diagnosis, or treatment. It is provided for your personal use and should be used for general informational purposes only.

In emergency or urgent situations, please dial 911, contact your medical provider directly, or go to the nearest emergency room.

When should I see a doctor?

If your symptoms suddenly get worse, or you develop symptoms that you would describe as severe, please contact a health care provider.

How do I self-quarantine? What precautions should I take?

Our infectious diseases experts recommend you follow these self-quarantine instructions if you have been exposed to someone with COVID-19 or are having symptoms.

Can I use this Symptom Tracker even if I don’t have symptoms?

Yes. Even if you don’t have symptoms, you can use the Symptom Tracker to monitor your health throughout a quarantine period.

How do I reset my password?

If you forgot your password:

  • Step 1: Visit tracker.nebraskamed.com
  • Step 2: Click the "log in" button
  • Step 3: Select the "Forgot your password?" link
  • Step 4: Enter the email address you used to register for Symptom Tracker
  • Step 5: Select "Send password reset link"
  • Step 6: We will email a link to you. When it arrives in your inbox, click the link, and follow the prompts to reset your password.
If you know your password, but want to update it:
  • Step 1: Visit tracker.nebraskamed.com
  • Step 2: Click the "log in" button
  • Step 3: Enter your email address and password, and submit
  • Step 4: In the upper right of the page, select your first name
  • Step 5: From the drop down menu, select "Change password." It's the second option down, under "Profile"
  • Step 6: Enter your current password, a new password, and confirm your new password
  • Step 7: Select "Save Password"

I think my Symptom Tracker account was hacked. What do I do?

Change your password immediately by following these steps:

  • Step 1: Visit tracker.nebraskamed.com
  • Step 2: Click the "log in" button
  • Step 3: Select the "Forgot your password?" link
  • Step 4: Enter the email address you used to register for Symptom Tracker
  • Step 5: Select "Send password reset link"
  • Step 6: We will email a link to you. When it arrives in your inbox, click the link, and follow the prompts to reset your password.
What kind of information will be on the certificate?

The certificate will:

  • State that you completed either a quarantine or isolation period
  • Confirm that it is safe for you to resume your normal day-to-day activities
  • Provide an option to include a summary of your symptoms

What happens if I don’t log symptoms every day?

You will still be eligible to receive a certificate. However, if you choose to include a summary of your symptoms on your certificate, there will be information missing.
